• How do I book a session? Call our office at (480) 459-9994.

• Do you offer video or phone appointments? Yes! We offer Telehealth appointments (video or phone) for our adult clients, upon request. We do not offer Telehealth appoints for children under the age of 15.

• Who will I meet with? Julie Jacobs is our primary provider. Learn more about Julie here. Jeff Fulkerson has limited availability at this time. Learn more about Jeff here.

• What are your rates? Windhorse Counseling is contracted with most insurance plans. Clients are responsible for their copay and deductible, if applicable. For self-pay clients, we charge $100 per 50-minute session.

• Can I reschedule my appointment? 24-hour notice is required to reschedule an appointment to respect the time of our other clients and our counselors. Any appointments cancelled without 24-hour notice will incur a $75.00 fee.

• Do I need to complete any forms before my first appointment? Upon scheduling your first appointment, you will be sent intake documents to complete before your first appointment. This helps your provider prepare for your appointment to make the most of your time.
